Step Two of Six: Medical Information

Congratulations!  If you have updated your child's information in Powerschool, you have completed step one of the online enrollment process.  

Step two of the online enrollment process involves your student's medical information. Click here to obtain the medical form for your child.  Please print the form, fill it out and return it to your child's school nurse as soon as possible.

Please also remember that your child is not enrolled until all immunizations are current and on record with your school.
