Allergy Action Plan - If your child has a history of allergies to food, environmental, and/or bee/wasp sting that require emergency treatment. The parent/guardian AND Physician is responsible for submitting the completed form as well as the prescription medication form to accompany any medications at school.
Asthma Action Plan - When a prescription medication to treat asthma is to be given/maintained at school to a student during the school day, the parent/guardian AND Physician is responsible for submitting the completed form as well as the prescription medication form to accompany any medications at school.
Diabetes Action Plan - The following forms and/or medications need to be completed by a parent/guardian and physician. Parent /guardian must make arrangements with the school nurse to meet one week prior to the first day of school or after diagnosis to develop an individualized health care plan to ensure the safety of your child while at school. Emergency medications such as glucagon, insulin, ketone strips, glucose tabs, and extra pump/insulin/glucometer supplies should also be provided.
Seizure Action Plan - If you child has a history of seizures, the parent/guardian AND Physician is responsible for submitting the completed form as well as the prescription medication form to accompany any medications at school.
